We continue to provide innovative care models, focused on improving quality and access to healthcare. **Responsibilities** The Disposition Planner for Mental Health is responsible for disposition planning, psychosocial and functional assessments, patient advocacy and education for patient and family. The Disposition Planner will provide resources, information and referrals that benefit adjustment to illness, hospitalization and recovery throughout the continuum of care. The Disposition Planner will provide appropriate communication/intervention based on specific age groups. **Qualifications** 1. **Required Minimum Education** :Masters Degree in Social Work or Graduate of an Accredited School of Nursing. 2. **Certification/Licensure:** LMSW (or eligible) in the state of Georgia or Licensed Registered Nurse in the state of Georgia. Certification in Case Management is desired. 3. **Required Minimum Experience:** Minimum 3 years of clinical practice/experience in general medical-surgical and/or disposition planning. 4. **Required Skills:** Organizational and professional communication skills a must. Ability toact as a liaison with multidisciplinary team members. 5. **Preferred Skills:** Knowledge of case management. Working knowledge of community resources.
