Overview Case Manager at the Bronx Citadel and Bronx Tremont Corps. These are Community Based Organizations located at 425 East 159thStreet, Bronx, New York, and 2121Washington Avenue, Bronx, New York, respectively. Under the direct supervision of the Corps Officers and the guidance of the Divisional Social Services Department, the Case Manager renders direct casework and/or group work services to individuals and families in need, including group members (of gangs, drug sets, etc.) and their families referred through the Ceasefire Initiative and Pathway of Hope participants. The Case Manager is responsible for assessing each individuals’ and family’s situation, providing services, referring to other agencies/services when necessary, and, in all cases, following-up all individuals/families served. Responsibilities + Partner with individuals and families toward their greater holistic well-being. + Extend compassionate, comprehensive support without discrimination. + Connect individuals to programs providing food, shelter, clothing, furniture. + Assist individuals in applying for benefits. + Expedite individuals’ access to detoxification and rehabilitation services. + Assist in arranging transportation. + Partner with other helping agencies in the community and make referrals. + Provide follow-up, including street and home outreach, to all individuals served. + Collect, analyze, and evaluate data. + Keep accurate and secure records – always guarding confidentiality. + Support Ceasefire Liaison in the provision of comprehensive services for and the relocation. of Ceasefire Initiative individuals and families. + Facilitate the Pathway of Hope poverty reduction initiative. Qualifications + Operate in conjunction with the Mission Statement of The Salvation Army. + Possess a Baccalaureate Degree in Human Services, related field, or comparable formative background. + Have experience in and aptitude for serving at-risk populations. + Demonstrate cultural competency. + Bilingual capability (English and Spanish). + Practice from a trauma-informed perspective.
