About the Job

+ Primary Purpose of Job Responsible to create systems to address health management of patients and ensure coordination of care. Provide direct medical services to patients including completion of nursing assessments and perform medical procedures according to acceptable standards of care. + Essential Functions • Supervise Medical Assistants to ensure that all medical services meet Department of Health regulatory standards and Charles Evans Center (CEC) clinical protocols. • Complete nursing assessments of each patient as assigned, including review of medical history, and active diagnoses; follows team approach to coordinate plan of care with physician. • Review all documentation completed by the treating physician and ensure all issues are addressed. • Assist in the patient examination and procedures, as required. • Perform all clinical procedures according to acceptable standards of care including but not limited to immunization administration, collection and processing of specimens, phlebotomy, and performing Electrocardiograms (EKG). • Provide patient education regarding all plans of care as necessary; ensure that patients understand all instructions provided by the treating physician. Provide copies of documentation as necessary. + Essential Functions cont’d • Design and establish monitoring systems that promote compliance with CEC clinical protocols and patient’s plan of care • Carry out triage protocols assessing incoming calls. • Review test results and correspondence to assess timeframes for proper follow-up by physicians. Assist physicians with prescription renewals and medication prior authorizations • Create Daily Assignment Sheet to ensure appropriate coverage of all clinical areas and cross coverage when necessary. . • Participate in the HSC Incident Review and Quality Management processes. Assist with clinical investigations for the purposes of quality improvement. • Monitor par levels of all supplies weekly and place orders for necessary immunizations, medications and exam room supplies. • Maintain collaborative relationships with the programs that support Health Services patients to ensure coordinated care. • Ensure all follow-up appointments are coordinated with the front desk staff. • Other duties, as assigned Job Requirements: Requirements • Bachelor’s degree preferred. • RN certification, currently licensed by the New York State Department of Education; including all mandated certifications. • Minimum four (4) years RN experience. • Knowledge of physician office/ clinic operating procedures. • Obtain and maintain Basic Life Support (BLS) certification. Additional Knowledge, Skills and Abilities + Excellent organizational skills and the ability to work on multiple tasks simultaneously. + Ability to work effectively in a customer service environment + Familiarity with medical office terminology and procedures + Ability to utilize verbal and communication skills to interpret physician information and advise patients and staff appropriately.
