The RN coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members\. The RN possesses good interpersonal and teamwork skills and practices the Core Values of the Mercy Health Care System\. SPECIFIC ACCOUNTABILITIES: **ASSESSMENT:** + In collaboration with the patient/resident/client and health team members, conducts initial and ongoing assessment, designed to gather data relevant to the patient’s/resident’s/client’s individual needs\. + In an accurate and timely manner, shall: 99. conducts and documents a nursing assessment of the health status of the patient/resident/client 99. collects objective and subjective data 99. modifies the data collected as the patient’s/resident’s/client’s status changes 99. reports assessment data as appropriate to other members of the health care team\. **ANALYSIS** : + Accurately and in a timely manner, analyzes the assessment data and establishes or modifies a nursing diagnosis to be used as a basis for nursing interventions\. **PLANNING:** + Accurately and in a timely manner, develops, maintains or modifies, the nursing component of the plan of care and communicates the nursing component of the plan of care to appropriate members of the health care team\. **IMPLEMENTATION:** + Accurately and in an timely manner, implements the nursing plan of care, which may include: 99. executing the current regimen prescribed; 99. giving direct nursing care; 99. assisting with the care of the patient/resident/client; 99. collaborating with other nurses and members of the health care team; 99. delegating delegable nursing duties; 99. providing patients/residents/clients with a safe and supportive environment by demonstrating an understanding of: + patient/resident/client rights, + confidentiality, + abuse and appropriate interventions, + Advance Directives; 99. assisting patients/residents/clients/significant others to obtain information and skills relevant to their health needs\. **EVALUATION:** + Evaluates the patient’s/resident’s/client’s response to nursing interventions + Documents and communicates the patient’s/resident’s/client’s response to nursing interventions to appropriate members of the healthcare team; + Reassess the patient’s/resident’s/client’s health status, revises the nursing diagnosis and makes changes in the nursing interventions as appropriate; + Involves, as appropriate, the patient/resident/client, family, significant others or other members of the health care team in the evaluation\. **MEDICATION ADMINISTRATION:** + Administers medications in a safe manner, which is consistent with the organizations policies and procedures and the State of Ohio Nurse Practice Act\. **PROFESSIONAL DEVELOPMENT:** + Maintains competence and licensure by participating in ongoing professional educational activities\. + Attends and participates in department/unit meetings per unit policy\. + Actively participates in performance improvement\. **EDUCATION, SPECIALIZED AND/OR TECHNICAL KNOWLEDGE REQUIREMENTS:** 99. Graduate of state approved school of nursing 99. Current RN licensure to practice nursing issued by the State of Ohio 99. Additional training in area of specialization required and demonstrated by unit\-based competency\. 99.
