MINIMUM RESPONSIBILITIES: The Bridges to Health Program: provides an array of individualized services that maintain the child in the community, by reducing the need for residential treatment and hospitalization of children with a serious emotional disturbance, developmental disability, medical fragility or involvement in the juvenile justice system. The Waiver Service Provider (WSP) will provide services to enrich and improve the life of the enrolled child. Waiver services can be offered to the child, the foster parents, the birth parents and other involved family members. Services are held primarily in the boroughs of New York City, in the child’s home and or community. Each WSP position is held on a per diem bases. Level 1 Services: * Skill Building for child and/or family/caregiver for successful functioning in the home and community * Day Habilitation Services for child when other skill building services are not appropriate, primarily for the developmentally disabled population * Planned Respite Services offers the family some personal time off where the child goes into the community with a skilled and able worker familiar with the child's special needs. The WSP will engage in activities which foster the child's various service goals * Family/Caregiver Supports and Services offers the family/caregiver concrete services as well as education on resources available to them to enhance the child's interaction with other children, family members and people in the community Level 2 Services: * Prevocational Services teaches concepts for success in a working environment * Supportive Employment Service support clients once their have procured paid work Level 3 Services * Special Need Community Advocacy and Support is implemented to facilitate communication with the school and community resources * Crisis Avoidance and Management and Traning Teaches psycho-education to address issues that disrupt the child and family * Immediate Crisis Response Services 24 hour services designed to respond immediately to crisis * Intensive In-Home Supports and Services provide interventions following a crisis * Crisis Respite short term relief for child and caregiver during a crisis MINIMUM QUALIFICATIONS: Depending on education and experience, applicants may be eligible to provide more than one level of service.
