PER DIEM/Hourly JOB Waiver Service Provider Position Bridges to Health Program/SCO Family of Services The Bridges to Health (B2H) program is designed to provide an array of individualized services that aim to maintain at risk youth in their community by reducing the need for residential treatment and hospitalization of children with a serious emotional disturbance, developmental delay, medical fragility or involvement in the juvenile justice system. The Waiver Service Provider (WSP) provides Medicaid waiver services to enrich and improve the lives of our at risk youth. Services can provided to the child, foster parents, birth parents and other family members and are provided in Nassau and Suffolk Counties in the child’s home and/or community. Candidates may apply for several or all skills indicated below should they meet the minimum job qualifications. Each WSP position is held on a per diem basis and can work up to 19 hours weekly Family Caregiver Supports & Services: Education and training for the family/caregiver so they can enhance the child’s interaction with other children, family members and people in the community Skill Building: Assist youth with socialization skills necessary for successful functioning in the home and community Day Habilitation Services: Assist youth with developmental disabilities with self-help, socialization and adaptive skills Planned Respite Services: Provide the family/caregiver with short term relief Prevocational/Supported Employment Services: Teach concepts (not job specific) related to employment opportunities in the community Special Needs Community Advocacy & Support: Provide educational support based on the child’s academic and behavioral needs as well as community services to minimize educational interruption Crisis Avoidance Management & Training/Immediate Crisis Response/Intensive In Home Supports/Crisis Respite: Psycho-education and training to prevent/address a crisis, 24 hour services to response to a crisis. • Preferred Qualifications: Master’s degree in Social Work, Psychology or in a related human services field, plus one year of applicable experience. • Minimum Qualification: Bachelor's Degree, plus one year of working with a child with a disability. • WSP applicants must have a valid Driver’s license with no recent major or multiple violations, reliable transportation, good communication and good writing skills.
