Travel and mileage are reimbursed. Payment is per diem. This candidate will serve as a consultant and not receive employment benefits. Job Description: The Bridges to Health Program is looking for a Registered Nurse to work with our foster children enrolled in the B2H Program through the Medically Fragile (MedF) Waiver. The nurse will be expected to meet with the child and family in their home every other month, to confer and collaborate with the Health Care Integrator (HCI) monthly, and to give consultation on how the child’s medical needs are met in services planning, safety planning, goal planning and transition planning. In 1966, Astor opened its first outpatient mental health clinic in Rhinebeck. Today, the agency operates a wide range of community-based behavioral health treatment programs as well as early childhood development and parenting programs at multiple locations in the Hudson Valley and in the Bronx, New York.
