About the Job

Conduct one face-to-face visit with the child every 2 months Document all home/community visits with the child on a progress note Attend the Initial Team Meeting & home visit Attend two additional Team Meetings annually Confer with the HCI at least once per month and document the same in a progress note Conduct an assessment and complete the Level of Care (LOC) Pediatric Patient Review Instrument (OCFS-8005C). Recommend appropriate service delivery goals Attend OCFS sponsored/required trainings Attend monthly staff meetings to stay current on practices and protocols. Attend individual/group supervision as requested by Program Director. Submit Service Summaries, Detailed Service Plans, Progress Notes, Logs, Expenses and any other necessary paperwork within required timeframes as specified by OCFS, LDSS, COA and SCO regulations, protocols and standards. Report incidents to the HCIA (Health Care Integration Agency) within 24 hours. Attend 11 mandatory trainings within the first 6 months of hire according to the following schedule: Prior to service delivery: * First Aid/ CPR o Mandated Reporting on Suspected Child Abuse and Neglect o Overview of B2H Waiver Program Documentation Requirements o CANS Training Within the first 6 months: HCI #4 Training Certified Registered Nurse -One year of service with children with disabilities -Outstanding writing skills -Familiarity with MS-Word, internet and email NYS Drivers License SCO Family of Services is an equal opportunity employer **Job:** **ICC CASEWORKER* **Organization:** **SD012* **Title:** *RN/Health Care Integrator* **Location:** *New York-Suffolk-Dix Hills* **Requisition ID:** *27995*
