All the primary duties within this document will be performed according to established policies, procedures and guidelines within the department. JOB SUMMARY: Responsible for coordinating home care services which includes establishing and maintaining the patient's plan of care. Responsible for the quality, customer service and financial management of assigned case load. PRIMARY JOB DUTIES: Assesses patient needs through the collection and interpretation of health care assessment data. Develops and ensures all patient plan of cares are appropriate to meet the individual needs of the patient. Coordinates patient care between the patient, caregiver, physician, primary case manager and other appropriate disciplines. Responsible for the completion of all home health documentation, including the timeliness of admissions, recerts, discharges, and all OASIS documentation. Ensures all clinical records are in compliance with health care law, reimbursement guidelines, agency policy and accreditation standards. Obtains, clarifies, documents and reports physician orders as appropriate. Provides patient and caregiver teaching as appropriate per the plan of care. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition. Identifies discharge planning needs and implements prior to discharge Conducts admissions, recerts, ROC, and discharges for the assigned case load. Conducts routine nursing visits as needed. May care for patients with specialized needs such as cardiac, pediatric, etc upon demonstrated competency. Conducts supervisory visits when necessary to monitor home health aide performance. Responsible for the quality outcome scores of the assigned patients. Responsible for the patient satisfaction scores of the assigned patients. Responsible for the financial management of all assigned cases including insurance authorizations and meeting reimbursement requirements. Demonstrates Standards of Performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. All other duties as assigned. ESSENTIAL JOB SPECIFICATIONS: 1. Education: ADN from approved school required. Bachelor's degree preferred. 2. Licensure: Registered nurse with current licensure to practice professional nursing in the state of North Carolina. BLS required. Valid Driver's License required. 3. Experience: Minimum 1 year of recent acute care, home health or related experience required. 4. Other - Hours of Work: Flexible hours to meet educational, departmental and institution needs. 5. Population Served: Neonates Infants Pediatrics Adolescents (12 – 17 years) Adults ( 18 – 71 years) Geriatrics ( 72 + years)
