LICENSURE/CERTIFICATION/REGISTRY/LISTING: LCSW - Licensed Clinical Social Worker Additional information on required licensure/certification/registry/listings * Requirement for behavioral health setting: Crisis Intervention Response Techniques (CIRT) or comparable approved certification prior to patient care assignment. This certification will be reviewed annually. ## Qualifications EXPERIENCE: Required: * 3-5 years of recent inpatient clinical social work experience 2 years post-LCSW clinical social work experience KNOWLEDGE, SKILLS, AND ABILITIES: Ability to multi-task and prioritize Analytical Skills Basic ability to use a computer and check email Communication - Accepts responsibility for effective two-way communication using clear and wellorganized oral and written techniques. PHYSICAL REQUIREMENTS: Light Work Exerting up to 20 pounds of force occasionally (up to 1/3 of the time), and/or; up to 10 pounds of force frequently (1/3 to 2/3 of the time), and/or; a negligible amount of force constantly (2/3 or more of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Assesses, coordinates and facilitates current and post-acute psychosocial, clinical and post-acute care transition needs for a select caseload of patients. Completes psychosocial assessments; provides crisis intervention, emotional support and short-term individual, family and group counseling with regard to diagnosis, treatment, and postdischarge plan utilizing principles of individual and family therapy theories; and creatively identifies and accesses resources to optimize effective and efficient patient care outcomes in collaboration with other members of the multidisciplinary team. MAJOR WORK ACTIVITIES: Completes psychosocial assessments. Provides clinical social work services to patients with acute medical and/or behavioral health needs. Identifies the patient's personal goals for hospitalization and discharge planning and applies information about patient's health behaviors, cultural influences, and belief or value system. Proactively seeks input from other members of the multidisciplinary team and identifies, facilitates and delivers appropriate clinical and psychosocial interventions accordingly. Formulates appropriate hospitalization and discharge planning goals and interventions in order to assist patients in achieving their personal goals. Identifies, addresses, and re-assesses based upon actual or potential barriers to treatment and/or discharge plan and works with healthcare team to address. Documents the patient's clinical status, plan of care and discharge plan according to CH policies and/or expectations of regulatory bodies. Mentors new CSW staff with regard to assessment skills and process, patients' rights, and ethical social work practice. 25 % Oversees provision of clinical social work services at assigned campus in collaboration with departmental leadership. Usually able to solve problems and suggest solutions that meet the customer's needs. 25 % Provides clinical social work interventions for a select caseload of inpatients, facilitating the clinical social work healthcare delivery process, addressing complex clinical issues, advocating for patients, family members, organization and profession, and contributing to the professional development of self and others. Performance consistently demonstrates everything outlined in the MWA and may at time exceed expectations. Seen as a valuable team member who normally requires little or no supervision. Usually able to solve problems and suggest solutions that meet the customer's needs. 25 % Provides clinical social work supervision to LCSWA (Licensed Clinical Social Work Associate) credentialed staff. Performance consistently demonstrates everything outlined in the MWA and may at time exceed expectations. Seen as a valuable team member who normally requires little or no supervision.
